1. Introductory remarks<\/h2>

\u200b<\/strong>1.1 Purpose<\/strong>\u200b<\/h3>

1.1.1.<\/strong> At Vestjyske Dyrl\u00e6ger, we will continuously process various personal data about you electronically and in other systematic ways in connection with your being a customer with us. This is necessary for you to become a customer with us so that we can treat your animals. In this connection, we are obliged to inform you which information about you we collect, register, disclose, or otherwise process.\u200b <\/p>

1.1.2<\/strong>. The overall purpose of processing your information is to be able to carry out the administration associated with consultations\/treatment for your animal(s), i.e. entering into the agreement, the start, course and completion of the treatment process, including in relation to services etc. to which you are entitled, as well as the obligations to which you are subject.\u200b <\/p>

1.2 Data controller and contract information<\/strong>\u200b<\/h3>

1.2.1<\/strong>. Vestjyske Dyrl\u00e6ger is the data controller for the information processed by us and is therefore, pursuant to data protection legislation, responsible for ensuring that your personal data is processed lawfully. <\/p>

1.2.2.<\/strong> For all the personal data mentioned, it applies that it is processed in accordance with the guidelines and framework set by data protection legislation and any other applicable legislation. <\/p>

1.2.3.<\/strong> This means, among other things, that you may at any time contact Vestjyske Dyrl\u00e6ger, Erhvervsparken 2, 6830 N\u00f8rre Nebel (tel. 70261680; email: vestjyske@vestjyske.dk) if you have questions about our processing of your personal data, or if you wish to exercise your rights under data protection legislation.\u200b<\/p>

2. Processing of personal data\u200b<\/h2>

2.1 Customer records<\/strong><\/h3>

2.1.1.<\/strong> When you contact us, we create a record for you and your animal(s) in our customer register (electronic IT system). Here we register a number of different personal data, which as a rule originates from you. <\/p>

2.1.2. We register, among other things, the identification number of your animal(s) and the owner\u2019s contact details, including the owner\u2019s name, private address, private and work telephone number, email address, and CPR number where necessary.In addition, medical record information about your animal(s) is continuously recorded. Finally, information about other special circumstances is recorded, such as insurance, credit agreements, health schemes, and agreements with other payers. <\/p>

2.1.3. The information in the system may be cross-referenced with information about other customers, e.g. to produce various types of statistics and summaries for internal use and upon request from public authorities.<\/p>

2.1.4. The legal basis for the processing is, as a rule, Section 6(1)(2) of the Danish Act on Processing of Personal Data (as of 25 May 2018, Article 6(1)(b) of the General Data Protection Regulation).\u200b <\/p>

2.2 X-ray and scan images<\/h3>

2.2.1. When X-ray and scan images are taken, we register your name and identification information about your animal(s) in this connection.<\/p>

2.2.2. The legal basis for the processing is, as a rule, Section 6(1)(2) of the Danish Act on Processing of Personal Data (as of 25 May 2018, Article 6(1)(b) of the General Data Protection Regulation.\u200b <\/p>

2.3 Prescribing, reporting and records of prescription-only medicinal products<\/h3>

2.3.1. Veterinary prescriptions contain your personal data, such as your name and address. It is stipulated by law which information, including personal data, a veterinary prescription must contain in order for a pharmacy to approve the issued veterinary prescription and for you as a customer to be able to obtain the prescribed medicine for your animal(s). Veterinary prescriptions are governed by the executive order on prescriptions and dose dispensing of medicinal products. <\/p>

2.3.2. If you have entered into an agreement with Vestjyske Dyrl\u00e6ger for us to be your herd veterinarian, personal data about you as well as identification information about your animals will be reported to the Veterinary Register (VetReg) at the Danish Veterinary and Food Administration. It is provided by law* that health advisory agreements for cattle, pigs and mink entered into between you and Vestjyske Dyrl\u00e6ger must be reported in the Veterinary Register (VetReg) at the Danish Veterinary and Food Administration. <\/p>

*) The legal basis is the executive order on health advisory agreements for cattle herds, the executive order on health advisory agreements for pig herds, and the executive order on mandatory health advisory services in mink farms.<\/p>

2.3.3. The legal basis for the processing is, as a rule, Section 6(1)(3) of the Danish Act on Processing of Personal Data (as of 25 May 2018, Article 6(1)(c) of the General Data Protection Regulation).\u200b <\/p>

2.4 Emails<\/h3>

\u200b<\/strong>2.4.1 We centrally log the emails sent from you to Vestjyske Dyrl\u00e6ger and sent from Vestjyske Dyrl\u00e6ger to you. This means that we register who the email is from, that it has been sent to or from you, and when the email was sent and received. The content of the emails you receive and send to Vestjyske Dyrl\u00e6ger will, as a rule, be recorded in your animal(s)\u2019 electronic medical record. <\/p>

2.4.2. As a customer of Vestjyske Dyrl\u00e6ger, you will\u2014if you consent\u2014receive reminders about agreed treatments or treatments that should be repeated by a specified date, such as revaccination.<\/p>

2.4.3. The legal basis for the processing is, as a rule, Section 6(1)(2) of the Danish Act on Processing of Personal Data (as of 25 May 2018, Article 6(1)(b) of the General Data Protection Regulation). <\/p>

2.5 Calendar system<\/h2>

2.5.1. In our electronic calendar, we record the appointments we have with the customer. In this connection, we state your name and, where applicable, identification information about your animal(s). <\/p>

2.5.2. The legal basis for the processing is, as a rule, Section 6(1)(2) of the Danish Act on Processing of Personal Data (as of 25 May 2018, Article 6(1)(b) of the General Data Protection Regulation).\u200b <\/p>

2.6 Laboratory samples and results<\/h3>

2.6.1. If samples are taken from your animal(s), biochemistry or other blood tests are performed, and parasite tests are performed on faeces, Vestjyske Dyrl\u00e6ger may itself be able to culture\/analyse results for some of the samples. If Vestjyske Dyrl\u00e6ger does not have the equipment to analyse the test result for a collected sample, the sample will be sent to an external laboratory in Denmark, EU\/EEA countries and possibly countries outside the EU\/EEA. When the sample is sent and the result is received, it will contain personal data about you, such as your name and the identification number of your animal(s). Your personal data will be processed in accordance with section 3.2 of this notice. <\/p>

2.6.2. The legal basis for the processing is, as a rule, Section 6(1)(2) of the Danish Act on Processing of Personal Data (as of 25 May 2018, Article 6(1)(b) of the General Data Protection Regulation).\u200b <\/p>

2.7. Registration of animals in registers<\/h3>

2.7.1. Registration of dogs in the Danish Dog Register. Pursuant to the executive order of the Danish Dogs Act (the Dogs Act), you are obliged to have your dog registered in the Danish Dog Register and to re-register your dog if you sell it, just as you must notify the Danish Dog Register upon your dog\u2019s death. If you enter into an agreement with Vestjyske Dyrl\u00e6ger for us to register your dog in the Danish Dog Register on your behalf, we will send personal data about you as well as identification information about your dog to the Danish Dog Register.\u200b <\/p>

2.7.2. Registration of catsIt is not mandatory by law in Denmark to have your cat registered. If you wish to have your cat registered, e.g. in the Danish Cat Register, and you enter into an agreement with Vestjyske Dyrl\u00e6ger for us to register your cat in the Danish Cat Register on your behalf, we will send personal data about you as well as identification information about your cat to the Danish Cat Register. <\/p>

2.7.3. Registration of horses with SEGES<\/h2>

All horses in Denmark must have a passport with medicine pages, pursuant to the executive order on identification of animals of the equine species. The executive order supplements EU Regulation 504\/2008 of 6 June 2008.SEGES is the body that issues horse passports in Denmark and thus registers horses.It is also mandatory by law to register a change of ownership of your horse and to register departure (dead horses) with SEGES.If Vestjyske Dyrl\u00e6ger on your behalf carries out or assists you with the above, we will use and send personal data about you as well as identification information about your horse to relevant persons, private companies and authorities. <\/p>

2.7.4. Registration of cattle, pigs, sheep and goats in the Central Husbandry Register (CHR)<\/p>

All herds of cattle, pigs, sheep and goats must be registered in CHR, pursuant to the executive order on registration of herds in CHR.Cattle, pigs, sheep and goats must be marked with specially approved ear tags. At the same time, the properties where the animals are kept must be registered in CHR.The animal owner\/farmer registers and deregisters their herd themselves at Landbrugsindberetning.dk.If Vestjyske Dyrl\u00e6ger on your behalf assists you with the above, we will use and, where applicable, send personal data about you as well as identification information about your animals to relevant persons, private companies and authorities. <\/p>

2.7.5. Legal basisThe legal basis for the processing is, as a rule, Section 6(1)(2) of the Danish Act on Processing of Personal Data (as of 25 May 2018, Article 6(1)(b) of the General Data Protection Regulation). <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t

2.8 Payment<\/h3>

2.8.1. Our system for handling payments is integrated with the customer register, which is why the processing of your personal data takes place in accordance with section 2.1 of this notice.<\/p>

2.8.2. If a credit agreement is entered into between you as a customer and Vestjyske Dyrl\u00e6ger, this will as a rule only be possible if you provide your CPR number. A consent form for processing the CPR number is completed and signed by the customer before the credit agreement is entered into. <\/p>

2.8.3. The legal basis for the processing is, as a rule, Section 6(1)(1-2) of the Danish Act on Processing of Personal Data (as of 25 May 2018, Article 6(1)(a-b) of the General Data Protection Regulation).\u200b <\/p>

3. Storage, transfer and disclosure, etc.<\/strong><\/p><\/div><\/div><\/div>

3.1. Storage of personal data<\/p>

3.1.1. Information about you is stored at the company\u2019s address.<\/p>

3.1.2. Certain information is also stored with our data processors in connection with operational and IT security tasks (such as external hosting of IT programs, software backup, etc.) that we have performed externally. Storage of the information with external partners (our data processors) is subject to Danish data protection rules, and data processing agreements have been entered into with data processors to ensure, among other things, that your information does not fall into the hands of unauthorised persons. <\/p>

3.1.3. Your personal data may also be entrusted to our lawyers or auditors for the purpose of carrying out a specific task. This may be the case, for example, if Vestjyske Dyrl\u00e6ger has used legal or auditing assistance in a matter in which you are involved, and where the relevant personal data therefore needs to be used by our auditor or lawyer.\u200b <\/p>

3.3. Disclosure<\/h3>

3.3.1. In connection with, among other things, reimbursement from insurance companies, reimbursement from municipalities, and disclosure of medical record material, including X-ray images and test results, to another veterinarian in connection with referral to, among other things, a specialist, or the customer\u2019s wish to change to another veterinarian, discharge summaries or the customer\u2019s medical record are disclosed, pursuant to the Danish Veterinary Association\u2019s Code of Ethics, to relevant public authorities and private companies as described above in this notice. The material will contain personal data about you as well as identification information about your animal(s). <\/p>

In addition, Vestjyske Dyrl\u00e6ger may disclose personal data about you and your animal(s) to our insurance company or other insurance companies if this is relevant and necessary for their processing of reported claims, etc.<\/p>

3.3.2. Your personal data may also be disclosed in connection with a potential purchase and sale of Vestjyske Dyrl\u00e6ger, including as part of the investigation and clarification process (due diligence) that may be carried out prior to any potential sale, etc.\u200b<\/p>

3.4. Deletion of data<\/h3>

3.4.1. Data protection legislation does not contain specific rules on when personal data must be deleted in customer relationships. This must be determined by Vestjyske Dyrl\u00e6ger as the data controller in each individual situation. In this assessment, particular emphasis must be placed on whether continued storage serves a legitimate purpose. <\/p>

3.4.2. In practice, the need to retain documentation of the history in a customer case\/customer medical record, including Vestjyske Dyrl\u00e6ger\u2019s ability to see which steps have previously been taken in the customer case\/customer medical record, is considered a legitimate purpose. The data Vestjyske Dyrl\u00e6ger uses to administer your customer relationship will therefore, as a rule, only be deleted at the earliest 5 years after the customer relationship has ended, as it is a legal requirement that prescriptions are stored for 5 years. In addition, the Danish Veterinary Association\u2019s Code of Ethics requires that original medical records, X-ray images and scan images, etc. belonging to the veterinarian who made the notes\/photographs should be retained for a minimum of 5 years. <\/p>

3.4.3. Other information about you that is not necessary to retain in order to administer your customer relationship will be deleted at the time when there is no longer any legitimate need for continued storage, or if legislation otherwise requires us to delete this information at a specific time.\u200b<\/p>

4. Your rights<\/h2>

4.1 Duty to provide information<\/h3>

4.1.1. Data protection legislation grants you various rights when we process personal data about you electronically. And the legislation imposes certain obligations on us\u2014such as informing you that we have collected or will collect personal data about you.\u200b <\/p>

4.1.2. If, in special cases, we process other personal data about you or carry out a different type of processing than what we describe here in this notice, Vestjyske Dyrl\u00e6ger will inform you separately of this.\u200b<\/p>

4.2 Access, rectification, etc.<\/strong>\u200b<\/h3>

4.2.1. You also have the right to be informed which personal data about you we have collected in connection with your customer relationship, and you have the right of access to the information.You have the right to request that we restrict, correct or delete personal data if you believe, for example, that it is incorrect or gives a misleading impression.<\/p>

4.2.2. As a rule, no law provides a statutory duty for a veterinary practice to keep medical records. If a veterinary practice is a member of the Veterinary Practices\u2019 Employers\u2019 Association under the Danish Veterinary Association, the veterinary practice must comply with the association\u2019s Code of Ethics, which states the following: <\/p>

\u201cIt is considered good ethics for the veterinary practice to prepare a medical record that can be used for quality assurance purposes and that can be used by the veterinarian\/veterinary practice in public and insurance-related matters.<\/em><\/p>

To provide a copy of the medical record, X-ray images, scan images, etc. to another veterinarian when, in specific cases, the veterinary practice is requested to do so.<\/em><\/p>

To provide a discharge summary, X-ray images, scan images, etc. to the animal\u2019s registered owner when, in specific cases, the veterinary practice is requested to do so.<\/em><\/p>

To refrain from providing a copy of the medical record, X-ray images or scan images, etc. to a layperson, unless it concerns the animal\u2019s owner as stated above.\u201d<\/em><\/p>

4.2.3. To the extent that you have given your consent to the processing of your personal data, you always have the option to withdraw it. However, Vestjyske Dyrl\u00e6ger may continue to process your personal data if Vestjyske Dyrl\u00e6ger has a basis for doing so.\u200b <\/p>

4.3 Further information<\/strong><\/h3>
